If you have a PPO  or a healthcare spending account HSA plan, massage may be a covered benefit, but only if you have a condition diagnosed by a physician or chiropractor that warrants massage therapy as a medical treatment.

The only way to have massage therapy with me and have your insurance billed for it, is to see me at Garza Chiropractic at 3303-C Kimber Drive in Newbury Park. I am only available at this location Monday 9am-12noon and Wednesday 2-5:30pm. 

You will first have to consult with Dr. John Garza to see if you are eligible or have a prescription from a physician specifically for massage therapy. He is licensed to bill insurance providers and I am not. I can not provide massage that is billed to your insurance outside of his office premises. If you receive massage at my Westlake Village location you are responsible for paying my regular fee check, cash, or Paypal. NO EXCEPTIONS.
